Attachment C - Per-Study Consent + Optional NDA Template

This document is presented to the creator at study acceptance and signed before the interview. It is recorded as a single consent ledger entry. If the study does not require an NDA, Section 8 is omitted from the version the creator signs.

1. What you are consenting to §

You are consenting to participate in one (1) research interview for the study above. Your participation is governed by the Creator Participation Terms (Attachment A) and the Creator Privacy Notice (Attachment B), which are incorporated by reference. If anything in this consent form conflicts with those documents, the stricter protection of the creator wins.

2. Recording §

Withdrawing recording consent. You may withdraw recording consent at any point during the interview. When you withdraw:

• Recording from the withdrawal point forward is not captured.
• The pre-withdrawal segment is handled per the option you select on this consent form: {{deliver_to_client | withhold_from_client | delete_entirely}} (default: withhold_from_client - Provider keeps the segment for fraud/compliance audit only and does not deliver it).
• If you do not state a preference, the default applies.

3. Identity disclosure §

• Provider will share your identity with the client as: {{full | masked | anonymized}}
• "Masked" means the client sees your screener answers and the interview content but not your handle, name, or contact data.
• "Anonymized" means even your screener answers are presented without identifying details.
• Live-session privacy controls (only when this study is masked or anonymized): Provider will protect your identity in the live session itself per SOW section 5b - Provider-issued calendar invite with a generic subject, Provider-provisioned meeting room with no creator-identifying tokens in the join link, your display name set to a Provider-issued alias, chat restricted to that alias, and identifying details bleeped/redacted from any recording delivered to the Client. For anonymized studies, Provider also conducts or co-hosts the call so a Client researcher does not directly hear or see you in real time, and offers voice modulation or transcript-only delivery; you pick one before the call.
• If the Client requires a session format that breaks these controls (e.g., Client insists on hosting the call themselves with your camera on), masked/anonymized cannot be used for that study and Provider will tell you before you accept.

4. Permitted uses by the client §

The client may use the interview content for the following purposes only:

• The client may not use the interview content for advertising, public marketing, training AI models, or resale, without a separate written release signed by you.

5. Retention §

• After the retention period expires, Provider deletes its copy and asks the client to delete theirs.

6. Payment §

7. Withdrawal after the interview §

You may withdraw consent for future use of the recording or transcript at any time by contacting {{privacy_contact_email}}.

What withdrawal does:

• Stops further use, copying, sharing, or AI processing of your recording and transcript by Provider going forward.
• Triggers a written request from Provider to the Client to honor the same limit, including deletion of any copies the Client controls if you request it.
• Does not retroactively undo uses the Client already made under this consent before withdrawal (e.g., a quote already incorporated into an internal Client report).
• Recording withdrawal during the interview is governed by section 2; this section governs withdrawal after the interview.

8. NDA (only included when {{nda_required}} = yes) §

In exchange for participation, you agree:

a. Confidential information means any non-public information disclosed by the client during the interview, including product roadmaps, pricing, unreleased features, business strategy, and any documents shared on screen. It does not include information you already knew, that becomes public through no fault of yours, or that you independently develop.

b. You will not disclose Confidential Information to any third party for {{nda_term, default 12 months}} from the interview date.

c. You may keep what you learned in your own head and use it for your own creative work; this NDA does not restrict your ongoing creative output, only the disclosure of specific Confidential Information shared during the interview.

d. You will not publicly post about this specific interview or its contents during the NDA term. You may say in general terms that you participated in paid research, without naming the client unless section 3 disclosed the client publicly.

e. Breach remedy is limited to actual damages plus injunctive relief; no liquidated damages clause applies to the creator.

9. Acknowledgment of identity reveal §

If the sponsor was masked at invitation and is revealed during the interview, you may end the interview at that point. If you stay, your acceptance of the reveal is logged.

10. AI-assisted analysis (per-study) §

• AI-assisted analysis allowed for this study: {{no | provider-hosted | client-hosted | named-third-party}}

• AI-assisted analysis means using an AI tool to help summarize, code, or draft reports from your interview content. It does not include training the tool on your data, which remains prohibited unless you sign a separate written release.
• Provider only uses AI tools whose terms forbid retention or model training on customer data; this is a hard rule and cannot be lifted on a per-study basis.
• If this study sets the field to "no," your interview content cannot be processed by AI tools by Provider or Client at any point.
